 Reporting a Work Injury 

  • Upon seeing or learning about any potential workplace injury, a First Report of Injury report needs to be completed. 

  • This report does NOT count against Safety Bingo.

  • All Departments Heads and Supervisors needs to clearly communicate with their staff to prevent injuries from not being disclosed and to ensure all injuries are properly reported. 

How to Report a Work Injury 

As soon as you learn that one of your employees has been injured, report the injury to SFM using one of the methods below. 

Please note that if it’s an emergency, call 911 or get the employee to an emergency care facility before reporting the injury to SFM. 

  • Report By Phone 

    • Call (855)675-3501 

    • Choose Option 1 to report-only and not speak with a nurse. 

    • If your employee is not sure whether to get medical treatment, choose Option 2 to speak with a nurse. 

      • Please note, all calls outside of normal business hours will automatically be answered by a nurse. Business Hours are 7am to 4pm M-F. 

  • Report Online 

  • Notify Human Resources 

    • Anytime an injury is reported, please notify the Human Resources department.
